ABSTRACT

Objective:To investigate the status of caregivers' benefit finding of senile dementia patients in Shiyan city, Hubei Province, and to analyze its influencing factors, so as to provide basis for carrying out targeted intervention measures.Methods:Totally 252 caregivers of senile dementia patients who visited Taihe Hospital, Shiyan People′s Hospital and Sinopharm Dongfeng General Hospital in Shiyan City, Hubei Province from July 2019 to July 2021 were selected as the research objects by convenience sampling method. The general Information Questionnaire, the Caregiver benefit finding questionnaire, the General self-efficacy Scale (GSES) and the Perceived Social Support Scale (PSSS) were used. The Pearson correlation analysis method was used for the correlation between caregivers' self-efficacy, social support and perceived benefit. Catreg was used to analyze the influencing factors of caregivers' benefit finding in senile dementia patients.Results:The total score of caregiver benefit questionnaire for 252 caregivers of senile dementia patients was (104.74 ± 14.82), the item "made me realize the significance of my health to my family and society" received the highest score (4.70 ± 0.55), while the item "made me quit bad habits" received the lowest score (3.22 ± 1.17). The GSES and PSSS scores of 252 caregivers of senile dementia patients were (24.74 ± 7.02) and (54.21 ± 13.32), which were positively correlated with the benefit finding ( r=0.565, 0.459, both P<0.01). The catreg analysis showed that the age of the caregiver, the average monthly income of the family, self-efficacy and social support were the influential factors of the perceived benefit level of the caregiver of senile dementia patients ( F values were 3.30-104.92, all P<0.05), which could explain 42.4% of the variance. Conclusions:The benefit finding of caregivers of senile dementia patients still needs to be improved. Medical staff should pay attention to the evaluation of benefit finding of caregivers of senile dementia patients, and provide personalized intervention measures to relieve caregivers' care pressure, maintain their physical and mental health, and improve the care quality of caregivers of senile dementia patients.

ABSTRACT

Objective To determine the accuracy of intracavitary electrocardiogram bimodal P-wave and characteristic P-wave in the position of PICC tip. Methods In the process of ECG guided catheterization, the morphological changes of P-wave in the intracavitary electrocardiogram were observed, and the bimodal P-wave and characteristic P-wave were recorded. The corresponding catheter length of the bimodal P-wave and characteristic P-wave were recorded. CT three-dimensional reconstruction was performed on 416 patients who underwent CT examination before and after catheterization. The distance between carina of trachea to superior vena cava and the right atrium junction was measured as best position of PICC and the standard distance. After catheterization, the distance from the tip of catheter to the corresponding catheter length of the bimodal P-wave and characteristic P-wave was measured as actual distance, and compared with the standard distance. Results In 416 cases, 168 patients (40.38%) were bimodal P-wave, and the accurate rate of bimodal P-wave was 86.31%(145/416), and the accurate rate of the characteristic P-wave was 13.69%(23/416). The standard distance was (41.96 ± 9.16) mm, the actual distance of corresponding characteristic P-wave was (48.00±15.18) mm and bimodal P-wave was (45.10± 11.16) mm. The distance of bimodal P-wave to control was (1.31±0.33) mm and characteristic P-wave was (6.00 ± 2.88) mm, which was significant statistical difference (t =5.197, P < 0.05), and the standard deviation of the distance of bimodal P-wave to control was smaller (0.33 < 2.88). Conclusions The intracavitary electrocardiogram bimodal P-wave is more accurate than characteristic P-wave in the position of PICC tip and it supplements and improves thePICC's tip position.

ABSTRACT

Objective To observe the effects of Yishen Tongluo Decoction (YTD) on the renal mRNA expression of transforming growth factor-β1 ( TGF-β1) and collagen Ⅳ ( ColⅣ) in membranous nephropathy ( MN) rats. Methods SD rats were randomly divided into normal group, model group, benazepril group (in the dosage of 10 mg·kg-1·d-1) , YTD group ( in the dosage of 20 g·kg-1·d-1) . The rats in various groups were given intragastric administration of corresponding agents. At the end of the fourth week, 24-hour urinary protein quantity, albumin ( ALB) , total protein ( TP) , triglyceride ( TG) , total cholesterol ( TC) , blood urea nitrogen ( BUN) , and creatinine (Cr) levels were observed. The mRNA expression levels of TGF-β1 and ColⅣ in renal tissue of rats were detected by immunofluorescence method, electron microscopy and real-time polymerase chain reaction (PCR) method. Results In the model group, urinary protein quantity in rats was increased, serum levels of TP and ALB were significantly lowered, serum levels of TC and TG were significantly increased, renal pathological changes were present, and mRNA expression levels of TGF-β1 and ColIV in renal tissue were up-regulated (P0.05) . Conclusion The therapeutic mechanism of YTD for MN is probably related with the inhibition of mRNA expression of TGF-β1 and ColⅣin renal tissue.

ABSTRACT

Objective To explore the clinical application value of serum procalcitonin (PCT) detection in diagnosis of neonatal infection disease.Methods The serum PCT,CRP values,routine blood test,blood cultures of 50 children with suspected infection was detected at admission and five days after admission.According to the clinical manifestations and the test results,50 children were divided into the bacterial infection group (32 cases) of and non-bacterial infection group (18 cases),and the value of PCT,CRP,routine blood test,blood culture was compared.Results Of bacterial infection group at admission,the serum PCT values in 27 cases were more than or equal with 2μg/L,white blood cell (WBC) increased in 6 cases,CRP of 7 cases were positive,blood cultures (3d) of 4 cases were positive,and of non-bacterial infection group at admission,the serum PCT value in 2 cases were more than or equal with 2μg/L,WBC increased in 2 cases,blood cultures (3d) of 1 case was positive ; Of bacterial infection group 5 days after admission,the serum PCT value in 23 cases were more than or equal with 2μg/L,WBC increased in 15 cases,CRP of 6 cases were positive,blood culture (5d) of 9 case were positive,and of non-bacterial infection group 5 days after admission,the serum PCT value in 1 cases was more than or equal with 2μg/L,WBC increased in 3 cases,CRP of 4 cases were positive,blood culture (5d) of 1 case was positive.At admission the cases of WBC increasing,CRP positive,blood culture positive of the two groups had no statistically significant differences (x2 =0.50,0.19,0.62,all P > 0.05),but the cases of the PCT value which was more than or equal with 2μg/L had statistically significant differences between the two groups (x2 =25.30,P < 0.05).5 days after admission the cases of WBC increasing,PCT,blood culture positive of the two groups had no statistically significant differences (x2 =4.39,20.30,8.56,all P < 0.05),but the cases of the CRP value significant differences between the two groups (x2 =0.83,P > 0.05).Conclusion The detection of serum PCT can be used as an important index of diagnosis for early neonatal infections,and its sensitivity and specificity are better than,blood routine and blood culture.

ABSTRACT

Objective:To study the interaction between the SARS-S1 protein and the SARS-sensitive cells.Methods:The SARS-S1 expression in the E.coli was purified and the anti-SARS-S1 was produced.After incubate with the SARS-S1 protein,cells was staining by the mouse-anti-SARS-S1 antibody and the anti-mouse antibody labeled by FITC.The result was determined by FACS.Results:After incubate with the SARS-S1 protein,different cell line have a different reaction to the SARS-S1 antibody.Conclusion:The SARS-S1 protein expressed by E.coli has a specific binding ability to the SARS-sensitive cells.
